The Cunniff-Dixon Foundation was created in 2005 in memory of Carley Cunniff and in recognition of Dr. Peter S. Dixon. Our mission is to enrich the Doctor-Patient relationship near the end of life by fostering human development in medicine and Palliative Care. Projects include Planning My Way, a free guide for end of life planning. Advance Care Planning is a process that helps you maintain your dignity and values in case you have to make difficult medical treatment decisions.
