Gundersen St. Elizabeth's Care Center Shields Ave. is an 80-bed skilled nursing facility. We provide holistic, person-centered care for residents who need short-term rehabilitative or long-term care, including a specialized Alzheimer's Unit for residents with dementia. Our personalized care plans include medical and therapeutic care, meaningful activities, social and family engagement in comfortable surroundings.
