About us
When you come to a Health Plus Wellness Center our doctors and staff will greet you with a warm welcome smile. Your first step will be to fill out our Personal Health History Questionnaire unless you filled it out on our website and printed it or emailed it before you came in. If you are coming to us to be treated for an auto accident injury, you will need to fill out the Auto Accident Questionnaire instead of the Personal Health History Questionnaire. You will need to provide your photo ID, and if you have coverage, we will need to see your insurance card to verify your benefits.
