Led by Psychiatric-Mental Health Nurse Practitioners (PMHNPs), this model brings together healthcare providers, mental health specialists, social workers, educators, and community resources to deliver personalized, holistic care that adapts to each person's evolving needs. With PMHNPs guiding treatment, the team coordinates care plans, fosters open communication, and promotes shared decision-making to address physical, emotional, and social well-being. This integrated approach ensures individuals and families receive seamless, high-quality support across the lifespan.
