Understanding the discomfort and shame associated with incontinence, we offer comprehensive home care for patients at home. Services include urinary catheter maintenance, a toileting program, emergency medical care, and a home accessibility assessment.

We provide a wide range of services for home intravenous therapy and nutritional support for patients with gastrointestinal disorders, congestive heart failure, cancer, or other conditions requiring medications or nutritional support through intravenous therapy. Our team includes nurses, physicians, pharmacists, and dietitians who collaborate to provide the most effective care for patients at home.

Our healthcare professionals conduct an initial in-home assessment to identify patient limitations, barriers, and safety concerns, and work with patients to develop a care plan that meets their needs. In collaboration with the patient's physician, our nurses develop a medication schedule, evaluate the effectiveness of medications, and inform the patient and/or caregiver about the correct dosage, frequency, and side effects.

Under the guidance of our certified wound, ostomy, and continence nurses, patients and their caregivers learn how to care for their ostomy and live independently. After a severe injury, such as a burn, stroke, or amputation, patients often require rehabilitation care to safely move from the hospital or treatment center to their home. Our nurse will assess the patient's abilities and then develop a care plan to meet their individual needs.

To help patients regain independence and recover from injury, New Horizon Home Health provides skilled nursing services. From medical treatment and illness management to wound care and physical therapy at home, we offer a wide range of services tailored to the unique needs of our patients.

Whether a patient needs physical, occupational or speech therapy, our team of highly qualified rehabilitation specialists helps patients improve strength, mobility, balance, speech and communication skills.
