A: After receiving a written prescription from your health care provider, you can call and make an appointment at one of our area clinics. To help expedite the admission process, our staff will take some information over the phone such as demographics, health insurance information, and method of payment. We will verify insurance benefits prior to your initial visit to the clinic. In order to serve you better, we have included a link on this page to the admission documents. After completing them in the comfort of your own home you can bring them in at the time of your first visit. We do file directly to your insurance carrier, however the patient is responsible for their co-payment, deductible, or non-covered amounts at the time of service. If there is a question of coverage, we will contact you to resolve any payment issues. We ask that you bring your referral or prescription with you on your first visit. In order to serve you better, we have included a link on this page to the New Patient Packet and Patient Referral Form. Q: Who will carry out the plan of care?
