About us
We help medically complex, chronically ill patients bridge the care from the hospital back to their doctor within 48 hours of discharge. Our team of 24 doctors, case managers and clinical pharmacist works exclusively by referral. Our goal is to improve the patient experience in the first 30 days of discharge to allow completion of the healing process and to stabilize their condition.
