About us

TRU PACE (Program of All-Inclusive Care for the Elderly) is a program of TRU Community Care, a Colorado-licensed, Medicare and Medicaid-certified nonprofit serving multiple Front Range counties (including Boulder, Broomfield, Adams, Jefferson, Arapahoe, Denver, and Weld, and beyond).
TRU PACE is designed to help older adults stay independent in their homes and community by providing a coordinated bundle of services—such as primary medical care, medication management, physical therapy, specialty services, in-home care, social work assistance, and transportation to and from the TRU PACE day center.
To enroll, participants generally must be 55+, live in the TRU PACE service area, be certified by the State of Colorado as needing nursing-home-level care, and be able to live safely in the community with PACE services.
